Follow-up between visits: what to measure in the first week

Three numbers tell you whether follow-up is working before the next visit: who logs, who goes quiet and who asks for help.

Why the first week?

It's when the patient has the most questions and when the habit of logging either forms or doesn't. Someone who logs nothing in the first days rarely starts later without someone reaching out.

What counts as logging?

What the patient does: the day's diary, a dose marked as taken, a photo sent. Opening the app doesn't count, and points earned don't either: points are a consequence and can come from something else.

Who is quiet?

Someone in care who hasn't logged anything in more than three days, or never logged during this episode of care. It's the week's call list, with those who never logged first.

How do you know if it improved over time?

By counting, at the end of each month, who was in care that day and how many had logged in the three days before. Today's status doesn't work for the past: someone discharged in August was still in care in July.
