What is AI hallucination?
It is a sentence the model generates that sounds right and has no source: a number, a rule or an instruction it made up.
Why it matters
In care, a made-up sentence becomes wrong guidance. A model that is right most of the time isn't enough: you need to see which sentence is backed.
